If you would like to save time at the office, you can fill out some of your paperwork in advance.
Patient Intake Forms and Questionnaire – These are forms that you will need to fill out for your appointment. If you would like to save time at your appointment, you can fill some of them out in advance using the my medical locker portal. You can find instructions on how to use the portal here. You can also ask questions, review results, and manage your accounts. For more help, just call or ask at your next appointment.
Release of Information Forms – If you have outside medical records that will need review for your appointment, such as imaging studies or surgical reports, you will need to sign a Release of Information form.
Likewise, if you want your consultation at Northwest Spine Surgery or other medical information to be sent to someone else, you will also have to sign a similar form.
